Une combinaison de m\u00e9dicaments peut \u00eatre prescrite pour am\u00e9liorer les chances de r\u00e9ussite de la th\u00e9rapie, car chaque agent s\u2019attaque au cancer de mani\u00e8re diff\u00e9rente. La chimioth\u00e9rapie administr\u00e9e en association avec des anticorps s\u2019appelle \u00ab chimio-immunoth\u00e9rapie \u00bb<\/p>\n<h3>Comment fonctionne la chimioth\u00e9rapie?<\/h3>\n<p>La chimioth\u00e9rapie est une th\u00e9rapie syst\u00e9mique, ce qui signifie qu\u2019elle affecte le corps tout entier. La chimioth\u00e9rapie cible et d\u00e9truit les cellules qui se divisent rapidement, comme les cellules canc\u00e9reuses. En raison de leur fonctionnement, les m\u00e9dicaments chimioth\u00e9rapeutiques peuvent \u00e9galement avoir le m\u00eame effet sur les cellules normales qui se divisent rapidement, dont celles des cheveux, des ongles, de la bouche et de l\u2019appareil digestif. Voil\u00e0 pourquoi la chimioth\u00e9rapie peut causer des effets secondaires, incluant la perte de cheveux, la diarrh\u00e9e, les naus\u00e9es et les vomissements.<\/p>\n<h3>Comment la chimioth\u00e9rapie est-elle administr\u00e9e?<\/h3>\n<p>Chaque dose de chimioth\u00e9rapie ne tue qu\u2019un pourcentage de cellules canc\u00e9reuses. Pour cette raison, on administre souvent la chimioth\u00e9rapie en plusieurs traitements afin de d\u00e9truire le plus de cellules canc\u00e9reuses possible.<\/p>\n<p>Chaque traitement de chimioth\u00e9rapie est g\u00e9n\u00e9ralement suivi d\u2019unep\u00e9riode de repos et de r\u00e9tablissement. Cette p\u00e9riode de traitement et de non-traitement est ce qu\u2019on appelle un cycle de chimioth\u00e9rapie.<\/p>\n<p>Les m\u00e9dicaments utilis\u00e9s en chimioth\u00e9rapie peuvent \u00eatre administr\u00e9s une seule fois par cycle ou plusieurs jours par cycle, selon le plan de traitement. Une cure compl\u00e8te de chimioth\u00e9rapie (nombre total de cycles de chimioth\u00e9rapie) peut durer plusieurs mois.<\/p>\n<p>Une s\u00e9ance de traitement typique comprend des pr\u00e9l\u00e8vements sanguins, une visite chez l\u2019oncologue ou une infirmi\u00e8re, suivie de lachimioth\u00e9rapie.<\/p>\n<p>Certains m\u00e9dicaments sont administr\u00e9s par voie orale sous forme de comprim\u00e9s ou de g\u00e9lules. D\u2019autres m\u00e9dicaments sont inject\u00e9sdirectement dans une veine \u00e0 l\u2019aide d\u2019une petite aiguille (injection ou perfusion intraveineuse).<\/p>\n<p>Si vous devez recevoir des m\u00e9dicaments par voie intraveineuse, votre m\u00e9decin peut vous conseiller la pose d\u2019un cath\u00e9ter veineux. En g\u00e9n\u00e9ral, ce proc\u00e9d\u00e9 s\u2019applique uniquement aux patients dont les veines sont petites et fragiles, rendant difficile l\u2019insertion d\u2019une aiguille intraveineuse normale dans une veine du bras.<\/p>\n<p>Un cath\u00e9ter veineux (un tube de plastique mince et flexible) est un dispositif introduit dans une veine pour faciliter l\u2019administration d\u2019un m\u00e9dicament. Il en existe plusieurs sortes, mais chacun fonctionne de la m\u00eame fa\u00e7on. Celui choisi pour vous d\u00e9pendra des pratiques normalement utilis\u00e9es \u00e0 votre h\u00f4pital.<\/p>\n<p id=\"tw-target-text\" class=\"tw-data-text tw-text-large tw-ta\" dir=\"ltr\" data-placeholder=\"Translation\"><span lang=\"fr\">Les m\u00e9dicaments de chimioth\u00e9rapie standard que les m\u00e9decins utilisent pour traiter la LLC, seuls ou en combinaison, comprennent:<\/span><\/p>\n<ul>\n<li>Fludarabine (Fludara\u00ae)<\/li>\n<li>Cyclophosphamide (Cytoxan\u00ae)<\/li>\n<li>Bendamustine (Treanda\u00ae)<\/li>\n<li>Chlorambucil (Leukeran\u00ae)<\/li>\n<li>Doxorubicin (Adriamycin\u00ae)<\/li>\n<li>Vincristine (Oncovin\u00ae)<\/li>\n<\/ul>\n<h3>Common chemotherapy regimens for CLL and SLL<\/h3>\n<p>Please note, some chemotherapy regimens also involve antibody therapy, like Rituximab or Obinutuzumab. \u00a0 <a title=\"CLL Antibody Therapy\" href=\"http:\/\/wp.lymphoma.ca\/lymphoma\/cll-sll\/treatment\/antibody-therapy\" target=\"_blank\" rel=\"noopener noreferrer\">You can find out more about antibody therapy here.<\/a><\/p>\n<p><strong>Fludarabine, Cyclophosphamide, and Rituximab (FCR)<\/strong><\/p>\n<p>FCR is usually recommended as the first treatment (also called induction or first-line treatment) for younger (less than 65-70 years old), fit patients and results in long-lasting disease remissions in many patients. FCR combination was the first therapy in CLL shown to significantly prolong overall survival (OS). Fludarabine and cyclophosphamide can be given as infusions into a vein or as tablets (pills), while rituximab is always given as an intravenous infusion. Older patients, particularly those who are frail or have poor kidney function, are unlikely to tolerate FCR well.<\/p>\n<p><strong>Chlorambucil and Obinutuzumab<\/strong><\/p>\n<p>In older patients with underlying health conditions (such as heart disease or poor kidney function), the combination of chlorambucil and obinutuzumab has proven to be a very effective therapy for CLL. When compared with chlorambucil alone, or the combination of chlorambucil and rituximab, chlorambucil and obinutuzumab proved to be the most effective at prolonging progression-free survival.<\/p>\n<p><strong>Bendamustine and Rituximab (BR)<\/strong><\/p>\n<p>BR can be used to treat older, fit patients if FCR is considered inappropriate. The side effects of bendamustine are generally milder than those commonly experienced with fludarabine.
