Les chercheurs continuent d\u2019\u00e9tudier les signaux des cellules (la communication de l\u2019information \u00e0 l\u2019int\u00e9rieur de la cellule et entre les cellules), un processus qui contribue \u00e0 la croissance et \u00e0 la survie des cellules canc\u00e9reuse. Certains m\u00e9dicaments sont actuellement cr\u00e9\u00e9s pour bloquer ces signaux et interrompre la croissance et la propagation des cellules touch\u00e9es par la LLC, tout en limitant les dommages caus\u00e9s aux cellules normales.<\/p>\n

Un grand nombre de nouvelles th\u00e9rapies cibl\u00e9es font actuellement l\u2019objet d\u2019essais cliniques, et certaines ont r\u00e9cemment \u00e9t\u00e9 approuv\u00e9es pour le
\ntraitement de la LLC. Parmi ces th\u00e9rapies cibl\u00e9es, plusieurs sont disponibles sous forme de comprim\u00e9s, permettant ainsi aux patients de prendre leurs m\u00e9dicaments
\n\u00e0 la maison.<\/p>\n

La plupart des th\u00e9rapies cibl\u00e9es affectent la moelle osseuse; il peut donc y avoir un risque d\u2019infections et de saignements. De nombreux patients \u00e9prouvent de la fatigue, parfois des naus\u00e9es ou des troubles intestinaux. Certains m\u00e9dicaments peuvent \u00e9galement \u00eatre la cause de neuropathies p\u00e9riph\u00e9riques (voir description sous \u00ab effets secondaires de la chimioth\u00e9rapie \u00bb).<\/p>\n

Currently, the drugs ibrutinib, idelalisib and venetoclax are the only targeted therapies approved by Health Canada for the treatment of patients with CLL.<\/p>\n

Ibrutinib (Imbruvica\u00ae)<\/h5>\n

Ibrutinib is a drug that attaches directly to a protein called Bruton’s tyrosine kinase (BTK), which is very active in CLL cells. BTK sends signals in the cell to help it grow and survive. Ibrutinib blocks the signals from BTK, which helps stop or slow down the division and survival of CLL cells.<\/p>\n

Ibrutinib comes in pills (tablets) that must be taken every day.<\/p>\n

Ibrutinib has been approved by Health Canada for treatment of the following patients with CLL:<\/p>\n

    \n
  1. Previously untreated CLL with 17p deletion.<\/li>\n
  2. Previously untreated CLL and not considered appropriate for treatment with a fludarabine-based regimen.<\/li>\n
  3. Those who have received at least one prior therapy and are not considered appropriate for treatment or re-treatment with a fludarabine-based regimen (ibrutinib alone or in combination with bendamustine and rituximab).<\/li>\n<\/ol>\n

    Ibrutinib is currently reimbursed by some public and private drug insurance programs in Canada. Janssen offers a patient support program for those being treated with ibrutinib and, in some cases, for those who need assitance to access ibrutinib.     Idelalisib (Zydelig\u00ae)<\/h5>\n

    Idelalisib is a drug that blocks the signals from a protein called phosphoinositide 3-kinase (PI3K)-delta. The main job of PI3K-delta is to send signals that help B cell grow, divide and survive. By blocking the signal from PI3K-delta, idelalisib helps stop or slow down the growth of CLL and SLL cells.<\/p>\n

    Idelalisib comes in pills (tablets) that must be taken every day.<\/p>\n

    Idelalisib has been approved by Health Canada for treatment of the following patients with CLL:<\/p>\n

      \n
    1. In combination with rituximab for those who have received at least one prior treatment.<\/li>\n<\/ol>\n

      Venetoclax (Venclexta\u00ae)<\/h5>\n

      Venetoclax is a drug that attaches to a protein called b-cell lymphoma 2 (Bcl-2). Bcl-2 plays an important role in keeping cells alive by blocking the actions of proteins that cause cell death. By attaching to Bcl-2, venetoclax stops Bcl-2 from inhibiting cell death.<\/p>\n

      Venetoclax comes in pills (tablets) that must be taken every day.<\/p>\n

      Venetoclax has been approved by Health Canada for treatment of the following patients with CLL:<\/p>\n

        \n
      1. Those with 17p deletion who have received at least one prior therapy.<\/li>\n
      2. Those without 17p deletion who have received at least one prior therapy and for whom there are no other available treatment options.<\/li>\n
      3. In combination with rituximab for those who have received at least one prior therapy.<\/li>\n<\/ol>\n

        Venetoclax monotherapy for 3rd-line treatment following BTKi failure is currently reimbursed by most public drug insurance programs in Canada, and is covered by some private insurance programs. For assistance with access to venetoclax, call the Abbvie Care Program: 1-844-346-6626<\/p>\n
