La th\u00e9rapie CAR T-cell est une th\u00e9rapie cellulaire prometteuse pour certains sous-types agressifs de lymphome, comme les lymphomes diffus \u00e0 grandes cellules B et les lymphomes \u00e0 cellules du manteau. Il s\u2019agit d\u2019une importante th\u00e9rapie qui donne de l\u2019espoir aux personnes dont les options th\u00e9rapeutiques sont limit\u00e9es.<\/p>\n

le cancer commence en 2017 apr\u00e8s l\u2019apparition de sympt\u00f4mes, dont certains sont l\u00e9gers mais aussi inqui\u00e9tants \u2013 des engourdissements dans certaines parties du corps qui finissent par dispara\u00eetre, une perte de poids que l\u2019on attribue \u00e0 un mode de vie plus sain, des sueurs nocturnes peu fr\u00e9quentes et des douleurs au cou et \u00e0 l\u2019\u00e9paule qui, finalement, se soldent par la d\u00e9couverte de quelques masses.<\/p>\n

Depuis environ un an, Gilles ressentait des sympt\u00f4mes, mais lui et sa femme les avaient attribu\u00e9s \u00e0 d\u2019autres causes.<\/p>\n

\u00ab C\u2019est au moment o\u00f9 j\u2019ai d\u00e9couvert les bosses que j\u2019ai compris que l\u2019\u00e9tat de Gilles \u00e9tait grave et, le lendemain, j\u2019ai pris un rendez-vous pour lui chez le m\u00e9decin. \u00bb [Gilles] \u2013<\/em> Francis<\/p>\n

Apr\u00e8s lui avoir fait faire des analyses de sang, le m\u00e9decin de famille de Gilles le dirige vers un oto-rhino-laryngologiste. Le sp\u00e9cialiste lui palpe le cou, recule d\u2019un pas et lui dit carr\u00e9ment : \u00ab Vous avez un cancer \u00bb. Il l’envoie alors en oncologie sans autres explications. Gilles et Francis sont stup\u00e9faits.<\/p>\n

Dans le centre anticanc\u00e9reux o\u00f9 l\u2019on dirige Gilles, un oncologue \u00e9tablit, \u00e0 l\u2019aide d\u2019une biopsie, qu’il est atteint d’un cancer tr\u00e8s agressif, un lymphome \u00e0 cellules du manteau. Ses traitements ne peuvent plus attendre. Il en a besoin imm\u00e9diatement.<\/p>\n

Gilles n’a pas le temps de se renseigner sur le diagnostic ou sur le sous-type de lymphome, pas le temps d\u2019assimiler mentalement la nouvelle pour se pr\u00e9parer au traitement, pas le temps de trouver le soutien dont il a besoin au moment o\u00f9 on lui annonce qu’il lui reste 18 mois \u00e0 vivre s’il n\u2019est pas trait\u00e9.<\/strong><\/span><\/p>\n

Peu d’options \u00e9tant disponibles pour traiter le lymphome \u00e0 cellules de manteau, l\u2019oncologue explique \u00e0 Gilles et \u00e0 Francis en quoi consiste le plan th\u00e9rapeutique.<\/p>\n

Ce traitement est offert dans un centre anticanc\u00e9reux, dont le plus proche se trouve \u00e0 220 km de la maison. Le voyage s\u2019av\u00e8re \u00e9puisant pour Gilles. \u00c0 son arriv\u00e9e, il est atteint d\u2019une jaunisse et se sent tr\u00e8s mal.<\/p>\n

Mais l\u2019important pour Gilles est de commencer imm\u00e9diatement son traitement. On lui administre une chimioth\u00e9rapie en association avec de l’immunoth\u00e9rapie. Malheureusement, l’immunoth\u00e9rapie provoque une grave r\u00e9action. Sa temp\u00e9rature monte en fl\u00e8che, sa tension art\u00e9rielle augmente, son rythme cardiaque devient irr\u00e9gulier et il a de gros frissons. Le traitement n’est pas chose facile.<\/strong><\/p>\n

Gilles doit interrompre temporairement la th\u00e9rapie, attendre d\u2019\u00eatre r\u00e9tabli avant de la reprendre. Il n’y a aucune pause entre ce traitement et le suivant, une greffe autologue de cellules souches.<\/p>\n

L\u00e0 encore, il n’y a pas de centre anticanc\u00e9reux assez proche pour effectuer l\u2019intervention, et Gilles et Francis doivent donc faire plus de cinq heures d’avion et de voiture pour venir \u00e0 Toronto.<\/p>\n

En avril 2018, on pr\u00e9l\u00e8ve les cellules souches de Gilles. Un processus \u00e9reintant. Il a fallu neuf jours de pr\u00e9l\u00e8vements continus pour obtenir suffisamment de cellules souches afin de le traiter.<\/p>\n

On lui pose ensuite un cath\u00e9ter de Hickman pour pr\u00e9parer la greffe qui doit avoir lieu \u00e0 l’h\u00f4pital non loin de sa r\u00e9sidence. Malheureusement, un accident se produit lors du changement d’un pansement, qui entra\u00eene une infection. On doit donc retirer le cath\u00e9ter de Hickman et retarder la greffe jusqu\u2019\u00e0 son r\u00e9tablissement avec la prise d\u2019antibiotiques.<\/p>\n

Apr\u00e8s un autre voyage \u00e0 Toronto pour r\u00e9installer le cath\u00e9ter de Hickman, on peut enfin proc\u00e9der \u00e0 la greffe de cellules souches en juillet.<\/p>\n

Gilles re\u00e7oit de la chimioth\u00e9rapie \u00e0 haute dose avant une r\u00e9injection de cellules souches. Il est tr\u00e8s malade. Apr\u00e8s la greffe, Gilles perd 45 livres, il a d\u2019importantes l\u00e9sions buccales, la nourriture a un go\u00fbt infect et tout sent mauvais.<\/p>\n

Gilles entame alors une th\u00e9rapie d’entretien con\u00e7ue pour favoriser la r\u00e9ussite d’un traitement primaire et pr\u00e9venir ainsi une rechute. Heureusement pour lui, tout se passe bien pendant pr\u00e8s de deux ans, puis il commence \u00e0 ressentir de fortes douleurs abdominales. Une tomodensitom\u00e9trie indique la pr\u00e9sence d\u2019une masse de la taille d’une balle de softball. Gilles est compl\u00e8tement abattu. Apr\u00e8s sa r\u00e9mission, il doit recommencer tous les traitements.<\/p>\n
